Who Is Entitled To Obtain A Certified Copy Of A Death Certificate? PDF Print E-mail

In order to obtain a death certificate, you must first establish your relationship to the decedent, pursuant to regulations established by the New Jersey State Registrar and Governor's Executive Order.

You will be asked to show documentation, in order to establish that you have a right to that document, in addition to your photo identification. The chart below will provide a list of who is eligible and the documentation to establish relationship.

Please bring original documents - photo copies of vital records are not acceptable as proof.

Child

Your birth certificate with your parent’s names showing; marriage license, if you are married to address your name change.

Grandchild

Birth certificate with parents' names to show relationship to decedent; your parents' birth (or death), certificates with their parents names showing; your marriage license, if you are married to address your name change.

Sibling

Your birth certificate with your parents' names showing; marriage license, if you are married to show name change current spouse your marriage license.

Current Spouse

Your birth certificate with your parents' names showing; marriage license, if you are married to show name change current spouse your marriage license.

Ex-Spouse

Not entitled unless you are obtaining same on behalf of a minor child - minor child's birth certificate with parents' names showing **  if you have remarried, you must bring your current marriage license to address the name changes.

Executor of Estate

Valid, surrogate's certificate if you are the: estate you will need to bring: with original signature and seal.

Step-Child

Not entitled.
