You will need to bring:

a) A fee of $10.00 per certified copy

b) The exact name of the decedent at time of death (first- middle-last

c) The exact place of the death (city) (We will only have it if death occurred in Toms River)

d) The exact date of the death (month-day-year)

a) The relationship to decedent. (We need to see your Birth Certificate with Parent's names on it showing relationship to deceased. We also need to see your Marriage License, if your name changed, due to a Marriage.)

b) The decedent's mother's maiden name

c) The decedent's father's name (when recorded)

d) A Valid Photo ID with current street address on it (not a P.O. Box) from the applicant.

e) Depending on the relationship to the decedent, other ID will be asked for to show relationship ** (Your birth certificate with parent's names on it, your marriage license, or other items to connect you to the decedent.)
