Overview:
The Registrar of Vital Statistics is the keeper of birth, death, marriage records, domestic partnership records and civil union records, also referred to as “ vital records.” These vital records are confidential and not openly accessible to the public. As per State Law, the Registrar is limited as to who he or she can issue to. A Vital record can be obtained in the municipality where the event occurred. The eligibility and necessary requirements are contained below.
If you are only available during lunch time, please call our Office prior to coming in to see us. If there is only one Registrar working, the office may be closed for lunch from 12:00 to 1:00 PM. The deadline is 11:45 AM in the morning or after 1:00 PM.
Requests for certified copies of vital records may be processed either in person at the above address, or the appropriate information, requirements and payment may be mailed to:

Fees
$10.00 for each Certified copy of a vital record
$28.00 for a marriage application
$10.00 for a Correction
All fees are payable by cash, check, or money order.

A valid and current photo driver's license or a valid and current County ID is acceptable, on its own.
Please note that "valid and current" means with the address where you live at the time of your application and not expired. If you do not have either one, then you need TWO alternate forms of identification:
Valid photo driver's license or photo non-driver's license with current address.
OR
Valid driver's license without photo and an alternate form of ID with current address.
OR
TWO alternate forms of ID are required, one of which must show the current address. |

Applying For Marriage/Civil Union License/Domestic Partnership: |

WHERE DO I APPLY?
If the ceremony is in New Jersey: The couple must apply
in the municipality where either the bride or groom or partner A or partner B legally reside.
If both reside outside of New Jersey: They must apply in the municipality where the ceremony will occur.

Marriage/Civil Union Application Requirements: |
|
|
|
HOURS FOR MARRIAGE LICENSE APPLICATIONS:
8:30 AM TO 11:45 AM OR 1:00 PM TO 3:45 PM
1) File for the license in the town where either APPLICANT is
a resident.
a) If the APPLICANTS are not residents of the State of
New Jersey, then you file in the town where the marriage/civil union is taking place. The license is then only good in that town.
b) Applicants and Witness need Valid Photo ID with
current address on it, (NOT P.O. Box) to show proof of
identity and where they are residents.

How can I obtain a vital record?
Per Governor's Executive Order 18, certified copies will only be issued if all the following criteria are met:
1. Complete an application (or send a letter) fully identifying the record with all pertinent information.
2. A certified copy of a vital record may only be issued to you, if you meet the relationship requirements per Executive Order 18 or pursuant to a Court Order.
3. A. Acceptable Identification, per NJ State regulations, is required.
Send a photo copy of Applicant's Identification, if applying by
mail. (Applicant is the person requesting the record.)

How Do I Obtain A Birth Certificate? |
|
|
|
|
You will need to bring:
a) A fee of $10.00 per certified copy
b) The exact name of the child at time of birth (first-middle-last)
c) The exact place of the birth (city) (We will only have it if birth occurred in Toms River)
d) The exact date of the birth (month-day-year)
e) The relationship to child
f) The mother's maiden name
g) The father's name (when recorded)
h) If name was changed, the corrected name is needed
i) A Valid Photo ID with current street address on it (not a P.O. Box) from the applicant. |

How Do I Obtain A Death Certificate? |

You will need to bring:
a) A fee of $10.00 per certified copy
b) The exact name of the decedent at time of death (first-
middle-last
c) The exact place of the death (city) (We will only have it if
death occurred in Toms River)
d) The exact date of the death (month-day-year)
a) The relationship to decedent.
(We need to see your Birth Certificate with Parent's names
on it showing relationship to deceased. We also need to
see your Marriage License, if your name changed, due

Who Is Entitled To Obtain A Certified Copy Of A Death Certificate? |

In order to obtain a death certificate, you must first establish your relationship to the decedent, pursuant to regulations established by the New Jersey State Registrar and Governor's Executive Order.
You will be asked to show documentation, in order to establish that you have a right to that document, in addition to your photo identification. The chart below will provide a list of who is eligible and the documentation to establish relationship.
Please bring original documents - photo copies of vital records are not acceptable as proof.

b) The exact name of each partner (first-middle-last)
c) The exact place of the domestic partnership (city)
(We will only have it if domestic partnership occurred

d) The exact date of the domestic partnership (month-day-

e) The relationship to the people in the domestic partnership
f) A Valid Photo ID with current street address on it

You will need to bring:
a) A fee of $10.00 per certified copy
b) The exact name of the Groom/Applicant "A" (first-
middle-last)
c) The exact name of the Bride/Applicant "B" (first-middle-
last)
d) The exact place of the marriage/civil union (city) (We
will only have it if marriage/civil union occurred in Toms River)
e) The bride's exact maiden name
f) The exact date of the marriage/civil union (month-day-
year)
g) The place of the marriage/civil union took place (street
address)
h) A Valid Photo ID with current street address on it (not a
P.O. Box) from the applicant. *Depending on the relationship to the couple, other ID will be requested, to establish the relationship. (Your birth certificate with parent's names on it, your marriage license, or any other documentation requested by Registrar.)
